Your Care at Home is a registered NDIS service provider, dedicated to providing personalised and flexible support services in the Sydney metropolitan area. Our mission is to empower NDIS participants and individuals living with disabilities to maintain their independence and carry out their day-to-day activities in the comfort of their own homes.
Our range of support services is designed to holistically address and improve one's physical, emotional, cultural, and social well-being, aiming to empower and preserve dignity while promoting independence.
Complimentary and free consultations are available to help you plan for your support needs, to assist you in maintaining your independence in living at home.
Assistance with daily activities, such as personal care, medication assistance and mobility.
Assisting with household and yard maintenance, lawn-mowing, rubbish removal and gardening.
Assistance in participating or accessing social and community activities, such as transport assistance and activity based transport.
Physiotherapist, psychologist, occupational therapy and therapy assist, assistive technology assessment, dietician and other allied health services.
Light house cleaning, dusting, vacuuming, mopping and laundry assistance.
Individual assessment and support by a nurse for clinical assistance.
